The purpose of this website is to provide the information that you need to make an informed decision about whether or not you would like to continue a provider relationship with me at the end of my tenure with Optum on July 30, 2026.

​Before diving into the information about seeing me in private practice, please know that I want you to make the decision that it best for you. Of course, I’d be delighted to continue seeing you after I leave Optum but I understand that there are many reasons that this might not be the best decision for you.
If you decide to continue care with Optum, I have complete confidence in the providers you would transfer your care to: Nurse Practitioner Billierose Hair or Dr. Enrica Basilico. I’m happy to talk with you about each of them if you want to know who might be a better fit for you if you decide not to continue seeing me.
This website is intended answer your questions about pricing, billing, insurance reimbursement, scheduling, appointment locations and timing, and other concerns you may have about continuing care in a different setting. Basic information is here and I will continue adding links with more detailed information.

Any questions or comments regarding seeing me outside of Optum should be handled with me directly via e-mail, text, or phone call. My patient care position at Optum ends at noon on July 30.

Non-Optum Appointments will be available with me beginning August 3, 2026. Please contact me directly to schedule.

Appointments will be via telehealth, through a HIPAA-compliant portal yet to be determined.

In addition to continuing to see Lifestyle Medicine patients, I will be working as an elementary school nurse on Monday, Tuesday, and Wednesday. I plan to see patients at mutually-agreed times on Thursday and Friday, as well as early-morning and late-afternoon appointments on Monday and Wednesday.

Patients using GLP medications will need to continue routine body composition (InBody) testing. This will take place at my home in Ballard. If Ballard is not convenient for you, we can explore alternate arrangements. You will not be able to continue InBody testing with Optum if you transfer care to me outside Optum.

Appointment fees are based on Medicare fee schedule for Seattle and will be

$100 for quick follow-ups (typically medication refills/dose increases). (code 99213)

$145 for longer appointments (medication change, extensive counseling, lab orders, etc). (code 99214)

Additional fees may be charged for time-consuming tasks performed outside an appointment such as insurance appeals. All potential additional charges will be discussed prior to performing services that may result in an extra charge. No surprise billing!

I will not be billing insurance but will provide a superbill for you to submit to your commercial (non-Medicare) insurance carrier for out-of-network coverage. This may be reimbursed or be applied to your deductible. You can contact your insurance company and ask about out of network coverage for the codes listed above.

I am not enrolling with Medicare. I am happy to see Medicare patients but they will not be able to submit for reimbursement for their appointments or other fees. FSA/HSA funds may be used. My research indicates that medications prescribed by me will be covered by Medicare part D, however this is not guaranteed.

Credit card (including FSA/HSA) card must be on file at time of appointment and will be charged when I complete all the documentation and any tasks associated with our appointment (usually later the same day). A superbill will be emailed to you when your card is charged.

Nutrition: as I begin seeing patients outside Optum, I will not have nutrition support. It is unclear whether or not patients seeing me would be able to continue seeing their nutritionist at Optum. If you’ve been invited to this website, you’ve likely been with me for long enough that your nutrition is well-managed. I hope be able to provide referral to nutrition services within a few months.
